Thank you
for using Hypertension
Plus. This instruction manual provides important information to help you
to get the most benefit from using the Hypertension Plus
service here.
If you
have any questions or concerns that are not answered by this manual, OMRON
Customer Service.
Intended Use and Intended User
The Hypertension Plus app
is intended to be used by patients of HCPs. The Hypertension
Plus app informs patients of HCP's decision regarding the patient's
medication plan. The HCP's decision is based on the patient's entered home
blood pressure measurement data and the defined patient treatment strategy
based on NICE guideline NG136 (Published date: 28
August 2019).
Compatible OS
Hypertension Plus app will work with iOS 13 or later/Android
OS8 or later.

Initial Settings
Account Activation
Use of Hypertension Plus
requires an invite from your doctor or other healthcare professional. If you
have not received an invitation to use Hypertension Plus,
ask your doctor for information on availability in your area.
Steps to
activate your Hypertension Plus account:
Step
1: Complete and submit the
registration form in the link provided by your
doctor.
Step
2: Your doctor will verify your
registration and complete your profile.
Step
3: You will receive an email
confirmation from OMRON
that your account has been created.
Step
4: Download Hypertension Plus and begin the
login process.
1 Tap [Create Account].
2 Review OMRON's
terms of service.
▪ In order for OMRON to be able to provide
the best user experience, use of Hypertension Plus
is subject to OMRON's terms of service. To use Hypertension Plus you must signal your agreement to the
terms of service. When you have signaled, tap [Next]
to continue.
3 Enter your email address, country code
and mobile phone number that is registered with your GP, then tap [Next].

4 Create your password.
▪ Enter and confirm your password, then tap [Next].

5 Confirm your information and tap [Send
SMS].
▪ Hypertension Plus will send an
SMS (text message) to the registered telephone number
for verification.
▪ Ensure that you have access to the registered telephone number and
tap [Send SMS].
6 Enter the verification code and tap [Confirm].
▪ When prompted, enter the verification number into the boxes on the
screen.

7 Congratulations! You have activated your Hypertension Plus account. Tap [Next]
to get started.

Start] to begin.
Daily Use
The Home Tab
The Home
Screen
The Home
Screen is the most important screen in Hypertension Plus. The Home
Screen shows tasks that you
need to complete today. To get the best results from Hypertension
Plus, make sure to complete all of the tasks each day.

1. Status Bar
Indicates your
measurement week progress. Tap the status bar to see detailed information about
your measurement week. See “Your
Measurement
Week [> 23]" to learn more.
2. Settings
Tap to open settings
screen. Refer to “Settings
[> 29]" chapter for more information.
3. Date Picker
Tap the date to select
a date from calender. Tap [<] or [>] to move to the previous or next day.
4. Tasks
Your daily tasks are
displayed here. Tasks with a checkbox, for example confirming that you have
taken medication, can be completed here on the Home Screen. Simply tap the grey
circle to mark it “done". Tasks that require further input are marked
with an arrow “>". Tap the arrow to open the task. See each task
in Hypertension Plus for more information.
5. Completed Tasks
When you complete a
task, it moves to the “Completed Tasks"
section. You can view or hide completed tasks by tapping the “Completed
Tasks" section.
6. Home Icon
Shows how many tasks
are still to be completed today.
Take Medication Task
The Take
Medication task will be displayed when you
have an active blood pressure medication plan. Your blood pressure medication
plan is created by your doctor and shared directly with you via Hypertension
Plus. After taking medication, mark the
activity complete by tapping the grey circle next to the task.

Measure Blood Pressure Task
During your
measurement week (see “Your Measurement
Week [> 23]“) the Home Screen will show the
“Measure blood pressure" task.

1 Tap [Add >]
next to the “Measure blood pressure" task.
2 Tap [Next].
To measure blood pressure accurately it is recommended to follow some simple
guidelines. Watch our short explanation video to learn more, then tap [Next].

3 Take the first blood pressure measurement
using your blood pressure monitor.
4 Enter the time of measurement, systolic/diastolic blood pressure reading and pulse rate. Then tap [Next].
5 Take the second blood pressure
measurement using your blood pressure monitor.

6 Enter the time of measurement,
systolic/diastolic blood pressure reading and pulse rate. Then tap [Next].
Note

7 Your average blood pressure from the
measurements is displayed along with a short explanation of the result.
Tap [Close]
to save the result and return to the Home screen.

8 The “Measure blood pressure"
task moves to the “Completed Tasks"
section of the Home screen and the measurement result is displayed.
▪ To view all of your recent blood pressure results, tap [Blood
Pressure].
Managing
Tasks
Hypertension Plus supports communication between you and
your doctor through specific tasks that appear on your Home screen. Tap the
task to activate.
The main
messages and tasks are described below.
Tell doctor how you are doing [> 17]
Doctor recommends the next medication step [> 17]
Update
medication schedule [> 18]
Tell doctor how you are doing:
At the
beginning of your
measurement
week [> 23] you will receive an action to tell your
doctor how you are doing.
Tap the
action to open a questionnaire which will ask
you about your use of medication in the past month, any symptoms or
side-effects that you would like to report to the doctor, and whether you have
made any changes to your lifestyle in the past month.
Your doctor
will use the information that you provide
in this questionnaire, along with the blood pressure data recorded during your
measurement week, to help decide if any changes should be made to your
prescribed medication.
Doctor recommends the next medication step:
Your doctor
may request that you move to the next step
in your medication plan. Typically this may happen if your blood pressure is
above target after your measurement week.
Tap the
message to see your current step and the
recommended new step. If you are happy to move to the next step, tap [Accept]. Your doctor will be informed and asked to
create your new prescription. To learn more about the recommended change, tap [Learn More].
After
reviewing the additional information, tap [Accept] to agree to move to
the next step. Your doctor
will be informed and asked to create your new prescription. If you wish to
reject the recommendation tap [Reject]. You will
then be asked to add a reason for your rejection. Please be as clear as
possible as this will help your doctor to better plan your medication.
After a
medication step change you will remain on the
current medication step. Please continue to take your medication as prescribed.
Update medication schedule:
Once the
prescription is available you will receive a
notification to update your medication schedule.
Collect your
prescription and then tap “Update medication schedule" to confirm the
change.
Tap [Next], then update reminders for the new
medication.
Tap [Update], then your
medication plan will be updated to the new step and you can update or set
reminders for the new medication. Your doctor will receive confirmation that
you have updated your schedule and moved to the next step in your medication
plan. Tap [Close] to go back to the Home screen. To
configure reminder again, tap [Reminders].

Medication Plan Change:
Your doctor
may recommend a change in medication plan,
for example after reviewing any reported symptoms or receiving feedback on a
rejected proposal. When your doctor recommends a change in plan, the Medication
Plan Change action will be shown. Tap the message to review the proposed
changes in your plan. Your doctor may have also added a personal message to
explain the reason for the change. If you are happy to move to the next step,
tap [Accept]. Your doctor will be informed and
asked to create your new prescription. To learn more about the recommended
change, tap [Learn More].
After
reviewing the additional information, tap [Accept] to agree to move to
the next step. Your doctor
will be informed and asked to create your new prescription. If you wish to
reject the recommendation tap [Reject]. You will
then be asked to add a reason for your rejection. Please be as clear as
possible as this will help your doctor to better plan your medication. After
rejecting a medication step change you will remain on the current medication
step. Please continue to take your medication as prescribed.
New Medication Plan
A medication
plan will normally have 3 medication
steps. If you are on the last step of your current plan, and your doctor
recommends a change in medication, your doctor will create a new medication
plan for you. Tap the task to view the new plan proposed by your doctor. Your
doctor may have also added a personal message to explain the new plan. If you
agree with the new plan, tap [Accept]. Your doctor
will be informed and asked to make a prescription for the first step of the new
plan. To learn more about the recommended change, tap [Learn
More].
After
reviewing the additional information, tap [Accept] to agree to move to
the next step. Your doctor
will be informed and asked to create your new prescription. If you wish to
reject the recommendation tap [Reject]. You will
then be asked to add a reason for your rejection. Please be as clear as
possible as this will help your doctor to better plan your medication. After
rejecting a medication step change you will remain on the current medication
step. Please continue to take your medication as prescribed.
Contact GP office
If your
doctor would like you to contact them, they
may send a request in Hypertension Plus. Tap the
task to view any personal message that your doctor may have added.
When you
have contacted your doctor, tap [I have contacted my doctor] to clear
the task from your
Home screen. If you are not able to contact your doctor at that time, tap [I
will contact my doctor later]. The message will stay
active in your task list.

2 Your Measurement Week [> 23]
Your Measurement Week
Your Measurement Week is
an important part of using Hypertension Plus to
manage your blood pressure. National guidelines in England recommend measuring
your blood pressure at home every morning and evening for one week in order to
get an accurate picture of your blood pressure. During your measurement week Hypertension Plus will alert you to measure your blood
pressure in the morning and the evening every day.
• What
happens with the data?
Taking measurements over 1 week gives an accurate picture of your
blood pressure. Your doctor uses this to review your treatment plan.
• What
happens if I miss a day?
Don't
worry, just continue measuring the next day. We will extend
your measurement week by up to 7 days to allow you to catch up!
• Why did
my measurement week stop before the end of the week?
Hypertension Plus automatically
calculates your average blood pressure after every
measurement. When your average blood pressure is either a lot higher or a lot
lower than your target, then less data is needed to confirm your blood pressure
status. Therefore Hypertension Plus may end your
measurement week early. Don't worry, your data will be shared with your doctor
in the usual way.
The status
bar at the top of the Home screen keeps you
up-to-date on the progress of your measurement week. Tap the status bar to
learn more about the measurement week.
Before your measurement week: The status bar tells you how many days
until your measurement week starts.
During your measurement week: The status bar keeps track of how many
measurement days you have completed and how many more need to be done.
After your measurement week: If you have successfully completed your
measurement week the status bar shows your average blood pressure result. Tap
the status bar to learn more about the result and what it may mean for you.
Your doctor will use this data, along with the questionnaire that you submitted
at the beginning your measurement week, to review your treatment plan. If you
did not take enough blood pressure measurements during your measurement week,
the status bar will indicate that your measurement week was not completed. When
a measurement week is not completed you will continue on your current
medication step until next month. Your doctor will be informed of the missed
week and may contact you directly.
The Blood Pressure Tab
Tap [Blood Pressure] to
enter the blood pressure tab. On the blood pressure tab you can:
• View your
blood pressure readings
• Enter blood
pressure readings
• See which
days you have taken readings in the past
• See which
days blood pressure readings are required as part of your
measurement week
View your blood pressure readings
• Tap a date
in the calendar to view your blood pressure readings from
that day.
• Use the
week / month toggle button at the top of the screen to
switch between a 7-day and full month view.
• Dates with
a dark blue filled circle mean that a blood pressure
measurement has been recorded on that date.
• Dates with
an unfilled circle mean that no blood pressure
measurement has been recorded on that date.
• Dates
highlighted with a light blue bar are part of your measurement
week. You should aim to take blood pressure measurements on these days (see
“Your
Measurement Week [> 23]"
page).
• Dates with
no highlight are not part of your measurement week. You
may still wish to measure and record your blood pressure on these days in order
to keep track of your health.
• All blood
pressure data entered into hypertension plus is shared
with your doctor.
• Tap [Graph] to show graph view of your
blood pressure readings. In the graph view, tap [Week]
to show each blood pressure reading in the week. Tap [Month]
to show the average of your blood pressure reading of each week in the month.
Tap [Year] to show the average of your blood
pressure reading of each month in the year.
• To delete a
reading, tap .

Use the
calendar to select the date for which you
would like to add a blood pressure measurement and tap “+" to open
the “Add Blood Pressure" screen. Complete the data when prompted.
Refer to “Measure Blood
Pressure Task [> 11]" page for further guidance on adding
blood pressure results.
The Medication Tab
Tap [Medication] to enter
the medication tab. From this tab you can:
• View your
daily medication schedule
• Mark
medication as taken
• See which
days you have taken medication in the past
• View your
medication plan
View your medication schedule
• Tap a date
in the calendar to view the medication scheduled for that
day
• Use the
week / month toggle button at the top of the screen to
switch between a 7-day and full month view
• Dates with
a green filled circle mean that you took all required
medication on that day
• Dates with
an unfilled circle mean that at least one medication dose
was missed on that day
Mark medication as taken
Use the
calendar to select the date for which you
would like to mark medication as taken. Tap the grey circle next to the
medication dose you would like to mark taken. A tick mark will be placed in the
grey circle and the message “Taken" will be added.
Edit taken medication
Use the
calendar to select the date for which you
would like to edit the taken medication. Tap the blue tick mark next to the
medication dose you would like to edit. The tick mark will be removed and the
medication will be classed as “not taken".
Your medication plan
Your
medication plan is displayed at the bottom of the
medication tab. The plan shows your current medication, and the next medication
step in your plan (if available). Tap “Learn More"
to view a more detailed overview of your medication plan. See “What is a
medication plan? [> 29]" below to learn more about medication
plans in Hypertension Plus.

What is a medication plan?
In England
high blood pressure is managed using a
step-by-step approach to medication. A low dose of medication is prescribed at
first, and then depending on the blood pressure response, higher doses, and
additional medications, may be required. The steps that should be taken are
very clearly defined by national guidelines for hypertension published by NICE.
A medication
plan sets out a series of medication
steps that may be taken if blood pressure is not controlled. In Hypertension
Plus a personalized 3-step plan is decided by your doctor and added to the
Hypertension Plus system. Each step shows the type and dosage level of
medication to be taken. You can view your personal 3-step plan in the
medication tab.
Your blood
pressure week is used to check whether your
current medication has lowered your blood pressure to below your target. If
your blood pressure is above target after your blood pressure week, your doctor
may recommend that you move to the next step in your medication plan. When you
move to the next step in your plan you will be issued a new prescription, and
your medication schedule will be updated in Hypertension
Plus.
If you reach
the end of your 3-step plan and your
blood pressure is not yet below your target, your doctor will likely create a new
3-step plan for you and ask you to start the new plan.
Managing
high blood pressure through this 3-step plan
approach has been shown to be safe and effective in multiple clinical trials,
with many people achieving lower blood pressure results than they had been
under the usual care with their doctor.
Settings
Editing
Your Profile
Through the
settings menu you can edit your phone
number and email address. Note that no other profile information can be edited
within the app.
1 Tap .
2 Tap “Account Settings“.
3 Tap the item that you would like to
change.
▪ You cannot change your name and date of birth information from
within the app. If there is an error in this information, contact your doctor.
4 Enter the new information and tap [Save].
Setting
and Editing Reminders
You can set
2 types of reminders within Hypertension Plus:
• Blood
pressure measurement reminders
• Medication
reminders
1 Tap .
2 Tap “Reminders“.
3 Tap the reminder time to edit the
schedule. Tap the slider next to the reminder time to toggle the reminder on or
off.
Blood
pressure measurement reminders are only
activated during your measurement week.
Medication
reminders can be set for up to 4 different
doses per day. Check the instructions on your medication prescription, or in
your medication plan on Hypertension Plus, to
confirm the number of doses per day of your medication, and the recommended time
of day for each dose.
Example:
If your
medication should be taken 2 times per day, in
the morning with breakfast and in the evening with dinner:
• set “Dose
1″ to breakfast time.
• set “Dose
2″ to dinner time.
• Confirm
that the sliders for doses 1 and 2 are showing
“on" (round button to the right and filled blue line).
• Confirm
that the sliders for doses 3 and 4 are showing
“off" (round button to the left and clear white line).
Hypertension Plus will now give 2 medication alerts; one in
the morning at your breakfast time, and one in the evening at your dinner time.

Refresh
Data
When you
have changed your smartphone or reinstalled
the app, you need to refresh data to synchronize your recorded data.
1 Tap .
2 Tap “Refresh
Data“.
3 Tap [Refresh].

Security Notes
“Hypertension Plus"
has the following security measures in place.
• 2 factor
identification: Your identity is verified by SMS (text
message) to your telephone number and email address when your account is
created;
• If you do
not use your account for 48 months your data will be
automatically deleted;
• All
communications between the app, doctor system and OMRON servers
are encrypted;
• Your data
is securely stored in encrypted form on OMRON's servers which have
been designed manage sensitive
personal data.
