1. Introduction: Why Accurate Paediatric BP Measurement Matters

Hypertension in children and adolescents is more prevalent than commonly assumed, yet it remains substantially under-recognised in routine practice. A significant proportion of this under-recognition stems not from a lack of clinical awareness, but from measurement error - an incorrectly sized cuff, a single hurried reading, or interpretation against the wrong reference range can each produce a misleading result in either direction.

Unlike in adults, where a single elevated reading is often the first data point in a well-understood pathway, paediatric BP interpretation depends entirely on accurate measurement being read against age-, sex-, and height-specific percentiles. An error of only a few millimetres of mercury, or a cuff one size too small, can be sufficient to shift a child across a percentile threshold and change the clinical picture entirely.

The consequences of getting this wrong run in both directions. Missed or delayed identification of true paediatric hypertension allows a modifiable cardiovascular risk factor to persist into adulthood, while false-positive elevated readings driven by poor technique can lead to unnecessary anxiety, investigation, and follow-up for the child and family.

2. Common Sources of Measurement Error in Children

Several factors specific to the paediatric population make accurate BP measurement more technically demanding than in adults, and awareness of these is the first step toward reducing error.

Cuff Size Mismatch

Cuff size is the single most common and most consequential source of error in paediatric BP measurement. A cuff that is too small for the child's arm circumference will systematically overestimate BP, while an oversized cuff will underestimate it. Because children's arm circumference varies enormously with age and body habitus, a single 'child-size' cuff used across all paediatric patients is a frequent and avoidable source of inaccurate readings.

Patient Anxiety and Positioning

Children, particularly younger children and those unfamiliar with clinical settings, are prone to anxiety-related BP elevation - a paediatric analogue of white-coat hypertension. Incorrect positioning is also common: feet unsupported and dangling, the arm unsupported or below heart level, or the child talking, crying, or moving during measurement can each meaningfully distort the reading.

Reliance on a Single Reading

A single office BP reading, taken in isolation, has limited reliability in children and should not be used alone to diagnose hypertension. Both transient anxiety-related elevation and measurement artefact are common enough that current guidance consistently recommends confirmation across multiple readings and, where relevant, multiple visits before a diagnosis is made.

3. Technique: Step-by-Step Correct Measurement

A standardised approach reduces variability and improves the reliability of paediatric BP readings across different clinicians and settings.

Preparation

  • Seat the child quietly for at least 3-5 minutes before measurement, with feet supported flat on the floor and back supported.

  • Position the arm at heart level, supported, with the antecubital fossa at the level of the right atrium.

  • Avoid measurement immediately after exercise, eating, or a stressful procedure (e.g. immunisation) earlier in the same visit.

Cuff Selection and Placement

Select a cuff in which the bladder width is approximately 40% of the mid-arm circumference and the bladder length covers 80-100% of the arm circumference. Measure mid-arm circumference directly where there is any doubt, rather than estimating by eye or age. The cuff should be placed snugly on the bare upper arm, with the bladder centred over the brachial artery.

Auscultatory vs Oscillometric Method

Oscillometric devices are widely used in routine practice for their ease of use and reduced observer dependency, and are acceptable for initial screening provided the device has been validated for use in children. However, current guidance recommends that any elevated oscillometric reading be confirmed by auscultatory measurement, which remains the reference standard for diagnostic purposes, using Korotkoff phase V for diastolic pressure.

Repeat-Reading Protocol

Take at least two readings, one to two minutes apart, and average them. Where the initial reading is elevated, repeat measurement within the same visit after a further rest period is recommended before treating the reading as representative, and confirmation on a subsequent separate visit is generally required before a diagnosis of hypertension is made.

4. Interpreting Results: Percentiles, Not Fixed Thresholds

Unlike in adults, there is no single numerical BP threshold that applies to all children. Normal BP rises with age and height throughout childhood, so results must be interpreted against reference tables or calculators stratified by age, sex, and height percentile.

Percentile-Based Categories

  • Normal BP: systolic and diastolic BP below the 90th percentile for age, sex, and height.

  • Elevated BP: BP at or above the 90th percentile but below the 95th percentile, or BP of 120/80 mmHg or higher, whichever is lower, in adolescents.

  • Stage 1 hypertension: BP at or above the 95th percentile up to defined upper thresholds, confirmed on repeated measurement.

  • Stage 2 hypertension: BP substantially above the 95th percentile, warranting more prompt evaluation and, where appropriate, specialist input.

Practical Implications

Because thresholds shift with age, sex, and height, using an adult cut-off (such as 140/90 mmHg) in a child - or applying an age-based estimate without checking height percentile - can miss clinically significant hypertension in a tall-for-age child or over-diagnose it in a short-for-age child. Reference to a current percentile table or validated digital calculator at the point of care is recommended rather than reliance on memorised approximate values.

5. When to Escalate: ABPM and Specialist Referral

Confirmed elevated office readings warrant a structured escalation pathway rather than immediate initiation of treatment based on office measurement alone.

Indications for Ambulatory Blood Pressure Monitoring

ABPM is recommended to confirm a diagnosis of hypertension in children with persistently elevated office readings, to assess for white-coat or masked hypertension, and to evaluate nocturnal dipping patterns. It is particularly valuable in children with risk factors for hypertension - obesity, chronic kidney disease, diabetes, or a history of prematurity - where office readings alone may not reliably characterise true BP burden.

Home Blood Pressure Monitoring

Home blood pressure monitoring (HBPM) offers a complementary approach, capturing readings in the child's usual environment and across multiple occasions, which helps reduce white-coat effect and provides a more representative picture of everyday BP than a single office visit. It can support both the diagnostic process and ongoing monitoring once elevated BP has been identified, provided families are given clear guidance on technique, timing, and use of a clinically validated device.

Red Flags Warranting Specialist Referral

  • Stage 2 hypertension at any age, or any hypertension in a child under 6 years without an evident cause.

  • Symptomatic hypertension (headache, visual disturbance, seizure) or evidence of end-organ involvement.

  • Suspected secondary hypertension, e.g. in the context of known renal, cardiac, or endocrine disease.

  • Significant discrepancy between arm and leg BP, raising suspicion of coarctation of the aorta.

Ongoing Monitoring

Children confirmed as having elevated BP or hypertension warrant a structured follow-up schedule, incorporating repeat in-clinic measurement, home or ambulatory monitoring where appropriate, and periodic reassessment of BMI, renal function, and other cardiovascular risk factors as part of a comprehensive management plan.

6. Conclusion

Accurate blood pressure measurement in children is technically more demanding than in adults, and small errors in technique - particularly cuff size - carry a disproportionate effect on the clinical picture given the reliance on percentile-based interpretation. Reducing measurement error through standardised technique, and interpreting results consistently against age-, sex-, and height-based references, is foundational to reliable identification of paediatric hypertension.

Key practice points for clinicians:

  • Confirm mid-arm circumference and select cuff size accordingly for every child; do not estimate by age alone.

  • Treat a single elevated oscillometric reading as a prompt for confirmation, not a diagnosis - repeat within the visit and confirm auscultatory where appropriate.

  • Interpret all results against current age-, sex-, and height-based percentile references rather than fixed or adult thresholds.

  • Escalate to ABPM or specialist referral where readings are persistently elevated, symptomatic, or occur in a child under 6 without an evident cause.
